Registration at Front Desk

The receptionist will call for a triage nurse and begin to fill out an information card to generate a record for your pet. Client information that is required includes: Your name; address; telephone number; alternate number; Driver’s license number. If this pet or your other pets have been to AEC in the past, let the receptionist know this. You will be given a patient history form to complete while you are waiting.
